Understanding your options

When additional support may help

A family may consider residential support when safe transfers, washing, dressing, toileting or mealtime assistance are difficult to manage at home. It may also be useful while the family prepares the home or arranges ongoing help. Hospital discharge does not automatically mean a particular care home is suitable; the branch must first review the person’s needs.

Everyday care priorities

Support shaped around individual needs

Use these areas to build a practical care discussion. Confirm the level of assistance and any specialist services with the selected branch before admission.

Mobility and transfers

Describe the help needed to stand, move between bed and chair or use a wheelchair. Share the therapist’s instructions and confirm the equipment and assistance available.

Personal care support

Plan support with bathing, dressing, grooming and toileting while preserving privacy. Encourage participation in tasks the person can safely manage.

Communication preferences

Tell the team about speech, understanding, hearing or vision difficulties. Share communication aids and allow time for the resident to express choices.

Mealtime assistance

Provide any swallowing assessment, food texture or fluid instructions from the hospital. Do not assume a standard meal plan is suitable for every stroke survivor.

Continuity of instructions

Bring the discharge summary, medicines list and follow-up appointments. Confirm how prescribed routines and changes in the person’s needs will be communicated.

Confidence and family involvement

Share meaningful daily goals and the resident’s preferences. Discuss visits, family contact and how agreed rehabilitation instructions fit into everyday care.

Questions families ask

Stroke recovery care FAQs

When should we enquire about care after a stroke?

Enquire while discharge is being planned where possible. Provide the hospital’s current care instructions so the branch can review suitability before a move is arranged.

Does residential care replace stroke rehabilitation?

No. Daily residential support does not replace a specialist rehabilitation plan. Confirm physiotherapy, speech therapy or other professional input separately, including availability and fees.

Can someone who uses a wheelchair be assessed?

Yes, discuss an assessment and explain transfer needs, equipment and any positioning instructions. Acceptance depends on whether the branch can safely provide the required support.

Can you support swallowing difficulties?

Share the clinician’s swallowing assessment and feeding instructions before admission. The branch must confirm that the required assistance and supervision are available.

How long does recovery take?

Recovery varies substantially. The treating healthcare team is best placed to discuss progress and goals; a care home should not promise a fixed recovery time or result.

What should the quotation include?

Ask for room charges, daily support, equipment, consumables, transport and therapy costs to be itemised. Confirm which services are included and which are arranged separately.
